An Important Role

The Geriatric Outpatient Clinic is directed by Mary Lyles, M.D. and is staffed by geriatricians and medical professionals of Wake Forest University Baptist Medical Center.  Patients and their families can have peace of mind knowing that the best possible care available is being provided.

This Clinic provides the following services:

  • Primary care for older adults in poor health and at high risk for hospitalization.
  • Consultative geriatric assessment for elderly patients with multiple medical and social problems. The goals of the consultative assessment are to serve the patient and his/her family; and to assist healthcare providers in their management of older patients with complex health conditions.
  • Coordination of outpatient follow-up procedures for patients discharged from the hospital inpatient services.

Clinic Visits

Scheduling Appointments
All patient visits at the Geriatric Outpatient Clinic are by appointment only. Appointments are scheduled from 8 a.m. to 5 p.m.  Dr. Mary Lyles sees patients 8 a.m. to 12 Noon Monday, Tuesday, Thursday and Friday.  Dr. Lyles also has afternoon hours from 1 p.m. to 5 p.m. on Friday.

Appointments may be scheduled by patients themselves, family members or primary care physicians.  Patients requiring an assessment of cognitive function, must be referred by a primary care physician.

Cancellations
If you are unable to keep your scheduled appointment, please call 336-713-8250 no later than 24 hours prior to your appointment so we may use this time for another patient.

Prescription Refills
Please call 336-713-8584 for prescription refills. Requests are taken during office hours Monday – Friday, 8:00 a.m. to 5:00 p.m., or by fax from the pharmacist at 336-713-8252.

Please contact your pharmacist to verify that your prescription has been called into the pharmacy and is ready for you to pick up.

Please note the following restrictions regarding prescription refills:

  • We do not refill prescriptions prescribed by other physicians.
  • We cannot refill pain medications after office hours or during the weekend.
  • If you haven’t been seen by our physician within the last six months, you must schedule a follow-up visit prior to prescription refill.  We want to make sure that the medication is properly dosed and that your health hasn’t changed.

If you want to leave a message for your physician, please call 336-713 8250 during business hours.  You will be called by the end of the business day.

After Hours Calls
If you have an urgent medical problem and you cannot wait to talk with your doctor during regular office hours, call 336-716-4849.  The on-call doctor for geriatrics will return your call.

Emergencies

If you have a medical emergency, go directly to the Wake Forest Baptist Medical Center Emergency Department or your nearest emergency room. Upon your arrival, notify the staff that you are a patient in the Geriatric Care Clinic and give your physician’s name.

Location

The Geriatric Outpatient Clinic is located in the Sticht Center on Aging, ground floor.  Patients check into the clinic through the Ambulatory Care Clinic.

Insurance

The Center accepts all patients who are insured, patients with Medicare/Medicaid coverage, thousand patients who have no insurance or income.

To Refer a Patient

If you are a physician or health care provider and you wish to refer a patient, or talk with a geriatrician please call 336-713-8250.

Following the patient visit, the physicians send a letter summarizing the findings and recommendations to the referring physician. 

After Hours Consultation

Medical providers who wish to speak with a geriatrician about a patient, patient transfer or medical question, can call the Physician Access Line toll free at 1-800-277 -7654 (716-7654 within Winston-Salem).  The on-call physician will return your phone call.
