House Call Program

The Geriatric Consultation Clinic is an interdisciplinary consultation clinic intended to assist primary care physicians to determine a course of treatment for older patients with multiple, complex health issues. Through this Clinic, primary care physicians have access to the resources of Wake Forest Baptist Medical Center on a consultative basis.

When referred to this clinic by their primary care physician, patients have a comprehensive evaluation and screening for common geriatric syndromes such as memory problems, difficulty walking, mood problems and falls. 

Physicians Have Access to National Leaders in Geriatric Medicine

For the referring physician, this consultative program allows them access to the highest level of specialization and medical expertise in our community with regard to frail older patients dealing with multiple complex health issues.

Health Conditions Evaluated in the Clinic

Common problems seen in the Geriatric Consultation Clinic include not only memory loss and other cognitive changes, but also a host of other geriatric syndromes including gait disorders, mood and behavioral disorders, pain syndromes, and polypharmacy. A social worker is available to assist the patients and their families identify and arrange appropriate senior resources, including resources for caregiver stress.

Clinic Visits

Scheduling Appointments

All patients seen at the Geriatric Consultation Clinic must be referred by their primary care physician. All Clinic visits are by appointment only. Appointments are scheduled from 9 a.m. to 12 p.m. and 1 p.m. to 5 p.m., Monday through Friday.

To Refer a Patient

If you are a physician or health care provider and wish to refer a patient or talk with a geriatrician, please call 336-713-8250.

Following the patient visit, the physicians send a letter summarizing the findings and recommendations to the referring physician.  For more urgent matters, the geriatric consultant communicates with the referring physician by phone. 

After Hours Consultation

Medical providers who wish to speak with a geriatrician about a patient, patient transfer or medical question, can call the Physician Access Line toll free at 1-800-277 -7654 (716-7654 within Winston-Salem).  The on-call physician will return your phone call. 

Cancellations

If your patient is unable to keep your scheduled appointment, please call no later than 24 hours prior to the appointment so we may use this time for another patient.

Insurance

The Clinic accepts all patients who are insured, patients with Medicare/Medicaid coverage, and patients who have no insurance or income.

Prescription Refills

If a geriatrician in the Consultation Clinic prescribes a new medication or modifies a patient’s existing prescription, medications may be provided by the Clinic.  All other prescriptions should be refilled by the primary care physician.

Calls and Messages
This clinic is available for consultation only.  Any urgent medical issue should be addressed with the patient’s primary care physicians.  Patients and families may call the clinic directly at 336-713-8250, and can expect a response within two days regarding the consultation.

This Clinic is available for consultation only.  Any urgent medical issue should be addressed with the patient’s primary care physicians. 

If you have a medical emergency, go directly to the Wake Forest Baptist Medical Center Emergency Department or your nearest emergency room. Upon your arrival, notify the staff that you are a patient in the Geriatric Consultation Clinic and give your physician’s name.

Location

The Geriatric Consultation Clinic is located in the Sticht Center on Aging, ground floor.  Patients check into the clinic through the Ambulatory Care Clinic.
