Kulynych Memory Assessment Clinic
The Kulynych Memory Assessment Clinic offers comprehensive assessments to older adults who are concerned about changes in their memory. The Clinic also offers assessments to those with mild to moderate memory impairment and who desire a second opinion.
Patients assessed in this Clinic undergo a comprehensive medical interview, physical examination and a standardized evaluation of memory and thinking abilities.
The Clinic, which is run by Kaycee M. Sink, M.D., M.S., offers patient care and conducts clinical research. It is one of the only memory assessment clinics in the country run by specialists in geriatric medicine. The Clinic has a team of experts who specialize in the diagnosis and treatment of memory disorders. This team includes Geriatric Medicine Faculty and Fellows, Pharmacists, and Neuropsychological Evaluators.
Clinic Visits
Scheduling Appointments
All patient visits at the Memory Assessment Clinic are by appointment only. Appointments are scheduled on Mondays from 12:30 p.m. to 5:00 p.m.
Appointments should be scheduled by a referring physician.

Cancellations
If you or your patient are unable to keep your scheduled appointment, please call 336-713-8250 no later than 24 hours prior to your appointment so we may use this time for another patient.
Prescription Refills
For medications that have been prescribed by the Memory Assessment Clinic, refill requests may be made by calling the clinic or having the pharmacy fax a refill request to 336-713-8800. Patients should allow 24 hours for prescription refills.
Information and Messages
For general information about the clinic or for appointment requests, patients and families may leave a message with the Coordinator of the Memory Assessment Clinic at 336-713-8963. Patients can expect a response on Monday or Wednesday.
For existing patients with health-related concerns, patients and families can call the clinic 336 713 8250 to speak with a nurse during normal business hours, Monday through Friday. Messages may also be left for physicians.
After Hours Calls
If you have an urgent medical problem and you cannot wait to talk with your doctor during regular office hours, call 336-716-4849. The on-call doctor for geriatrics will return your call.
Emergencies
If you have a medical emergency, go directly to the Wake Forest Baptist Medical Center Emergency Department or your nearest emergency room. Upon your arrival, notify the staff that you are a patient in the Memory Assessment Clinic and give your physician’s name.
Location
The Kulynych Memory Assessment Clinic is located in the Sticht Center on Aging, ground floor. Patients check into the clinic through the Ambulatory Care Clinic.
Insurance
The Clinic accepts all patients who are insured, patients with Medicare/Medicaid coverage, and patients who have no insurance or income.
Primary Care
The memory assessment clinic is a consultative clinic and does not manage a patient’s primary care. Following the patient visit, recommendations will be made to the referring primary care physician.
To Refer a Patient
If you are a physician or health care provider and you wish to refer a patient, please call 336-713-8963.
Following the patient visit, physicians send a consultation letter summarizing the findings and recommendations to the referring physician.
After Hours Consultation
Medical providers who wish to speak with a geriatrician about a patient, patient transfer or medical question, can call the Physician Access Line toll free at 1-800-277-7654 (716-7654 within Winston-Salem). The on-call physician will return your phone call.
