Prior to Leaving the Institution
You will be receiving information on the extension of your benefits from NCBH Benefits Office. Watch for this important information.

  • Please turn in your ID badge to NCBH Human Resource Office (3rd Floor Meads Hall). Your last paycheck can not be deposited until your badge has been turned in.
  • Make sure you have returned all items to the Library.
  • Complete all medical records with the Medical Records Department. Please print the attached form and present to Medical Records for verification of completion of all records.
  • Return beepers to your individual residency program.
  • Remove content and locks from any lockers you may have.
  • Provide forwarding address to House Staff Office, NCBH Human Resources and the Mailroom by filling out the change of address form.
  • Review Checklist for Benefits
