Clinical Neurophysiology Fellowship Application

Dear Applicant:                                              

Thank you for your interest in the Clinical Neurophysiology Fellowship Program at Wake Forest University School of Medicine.  We offer four clinical neurophysiology fellowship positions per year.  There are three separate pathways: EEG/Epilepsy, EMG Neuromuscular, and Sleep Medicine.

Prerequisite:

Each applicant is required to have completed or will complete a three-year adult neurology or pediatric neurology residency program accredited in the United States or Canada.  Applicants must qualify for licensure by the North Carolina Board of Medical Examiners and must be Board Eligible for the American Board of Psychiatry and Neurology.

International medical graduates must hold a J-1 visa or have resident alien status and they must have a valid ECFMG certificate.

How to Apply:

Please submit the Clinical Neurophysiology Fellowship application (located above), your letter of interest, current curriculum vita, and three letters of recommendation (one letter must be from the director of your core residency program) to:

Once your application materials have been received and evaluated, competitive candidates will be contacted directly for an interview.  All applications should be received by June 2008, for the 2009 positions.  Applicants are encouraged to submit their information as soon as possible since earlier invitations to interview will allow for greater flexibility in scheduling and there are limited interview slots available.
