2009 Registration Information
A completed registration form is required for all ACLS and PALS classes.
For BCLS classes please call Life Support Education at 716-2800
Hospital Employees
A current BLS card must be sent with all registrations
and a copy of your ACLS/PALS card for a refresher
course
School of Medicine Employees
A completed WFUSM Authorization Form must accompany your
registration in addition to a current copy of your current BLS card
for all registrations and a copy of your ACLS/PALS card for
all refresher courses
Non Medical Center Personnel
A current BLS card must be sent with all registrations
and a copy of your ACLS/PALS card for a refresher
course
