1.

Introduction - Welcome to The Family Medicine Center and Wake Forest University Family Physicians.  We are pleased that you have chosen us for your medical care and are looking forward to providing the best possible comprehensive health care for you and your family.

The Family Medicine Center is the out-patient care and teaching facility of the Department of Family and Community Medicine of Wake Forest University School of Medicine.  To help you learn more about our services, please take a few moments to read the information that follows.

Family Medicine - We are specialists in Family Medicine, “the medical specialty which provides continuing and comprehensive health care for the individual and the family.”

In addition to the treatment of physical and emotional illnesses and injuries, the Family Medicine Center also provides:

Well baby and child care

Obstetrical and gynecological care

Insurance, school, and sports exams

Cardiac exercise testing

 

*

Minor surgery including skin biopsies and vasectomies

Fitness and dietary guidance

Health and disease patient education

 

*

Individual, couple and family therapy

 

Laboratory tests and routine x-rays are also available on the premises.

Diabetes Education

Comprehensive Geriatric Assessment Team

 

 

We continue to take care of our patients while they are in the hospital.  Depending on the nature and seriousness of the problem, you will receive care either from our Family Medicine inpatient team or from one of our subspecialty colleagues

The Modules - Module A, B, C, or D -  where your provider, physician, or physician assistant sees you - is his/her office in The Family Medicine Center. Each module has its own doctors, nurses, and receptionists.

Continuity of care means regular care from health professionals who know you and your family’s medical history. Continuity is very important to your health and to us. We always try to arrange for you and your family to be seen by your personal physician(s). If that is not possible, we will try to have another physician in your regular module care for you. We hope that you and the staff in the module will get to know each other, so that we will be even better able to serve you.

 

 

4.

Appointments - To make an appointment, please call The Family Medicine Center at (336) 716-4479. When you call, be prepared to give your physician’s name, your birth date, and your insurance information. Knowing the reason for you visit will allow the operators to properly schedule your appointment.

Hours of Operation

Monday – Friday, 8:00 a.m. to 5:00 p.m.

Thursday, 5:30 p.m. to 9:00 p.m.

Saturday (emergencies only), 9:00 a.m. to Noon


Individual providers are not available every day for patient appointments because of their teaching and research responsibilities.  If you have an illness or injury that cannot wait until your provider is available, arrangements will be made for you to be seen by another physician at the Family Medicine Center.  Some of our providers are Family Medicine residents and Physicians Assistants who have faculty members immediately available on-site and who supervise your care.  In keeping with the educational mission of our institution, occasionally a medical student may see you before your physician comes in the room.

 

 

5.

Cancellations and Missed Appointments - When unexpected circumstances will not allow you to keep a scheduled appointment, please call us to cancel as soon as possible. If you miss an appointment without calling us to cancel it, we are unable to serve another patient at that time, which makes it difficult for us to provide our patients with the medical care they deserve.  Repeatedly missing appointments can also make it difficult for us to continue providing the care you deserve.

Minor Emergency and Urgent Care - During regular business hours, our operators will assist you in getting an appropriate appointment, or speaking with our Triage Nurse.  However, there is always a doctor—our “pool doctor”—available to see patients with more urgent and serious problems when appointments are not available on the regular schedules.  On Saturday mornings, you may be asked to come into our emergency clinic where doctors and staff are available to care for those who need to be seen.

 

 

7.

On-Call - When the office is closed, calling (336) 716-4479 will connect you to our answering service.  They will give your message to the on-call doctor who will call you back.  Please be patient if this takes a while because the doctor is also caring for patients in the hospital and emergency room!  If our on-call doctor recommends that you go to the Baptist Hospital/Wake Forest University Baptist Medical Center Emergency Room, upon arrival let the nurse there know that you are a Family Medicine patient.

We can only help with prescription refills and appointments during our regular office hours.

 

 

8.

Messages: Talking to Your Doctor or Nurse - When you leave a message our goal is to quickly respond.  However, our providers and nurses often have a difficult time breaking away from seeing patients and their other responsibilities.  Therefore, soon after your message is left, our triage nurses will review and if possible respond back to you.  If you would rather wait and only speak with your physician or nurse, please inform the operator, but realize that it can be 24 to 48 hours before they will be able to call you back.

 

9.

Prescription Refills - It is important to call for a refill at least two to three days in advance.  If you need a prescription refilled, it is faster to have your pharmacist contact us, but even then, it can take 48 hours to respond to refill requests.

 

10.

Test Results - As soon as test results are received, we will call you if there is a problem.  Otherwise, you will receive the results in the mail within three weeks.

Our Internet Site - At www.wfubmc.edu/fam_med, you can find out more about the Family Medicine Center, our department, common medical problems, and links to recommended health care resources on the internet.

 

12.

Billing - It is very important that you have your current address, phone number, and insurance card with you when you check in.  If a payment or co-payment is due prior to being seen, it will be collected when you check in.  Our billing and collections are provided through Wake Forest University Physicians’ Business Office.  If you have any questions about your Family Medicine Center account, please call (336) 716-3988.

 

13.

Location and Parking - The Family Medicine Center is located on the third floor of the Piedmont Plaza I building, which is in the southeast corner of the intersection of Stratford Road, Country Club Road, First Street and Miller Street, commonly known as Five Points.  Free parking is available in front of the building, with access from First Street or Miller Street.
