Campus-Specific Instructions  

For the Bowman Gray Campus or Reynolda Campus, fax your orders to the ARP office at 713-7395,  email them to Jan Snyder at arpoffice@wfubmc.edu  or deliver them directly to the office.

For the Downtown Campus, fax your orders to 713-1177, email them to Janet Harvey at arpoffice@wfubmc.edu  or deliver them directly to the office on the ground floor of the A1 Building.

For the Friedberg Campus, fax your orders to 716-1501, email them to Patricia Wood at arpoffice@wfubmc.edu or deliver them directly to the office. 

Animal Order Form

General Information:

Before ordering any animals, the Principal Investigator must have an Institutional Animal Care and Use Committee (IACUC) approved protocol in place. The Animal Order Form then needs to be filled out completely and forwarded to the Animal Resources Program (ARP) office.

Order forms must be complete with ALL information.  Be sure all signatures are in place, protocol numbers and account numbers are correct, and that a contact phone number, and the principal investigator’s name are on the form as well.

Your order could be delayed if the information on the form is incorrect or incomplete.

If you are ordering multiple strains, mixed genders, or ordering animals for more than one protocol or on multiple account numbers,  you must fill out a separate order form for each.

Please note: Vendors may need more than one week to fill orders for large quantities, timed-pregnant animals, exact age or weight requirements, or unique strains.  Consider a standing order if you will need the same number, gender, and strain of animals delivered over a period of time. This will help establish a timely and reliable delivery of your animals.  If you have questions about establishing a standing order, ARP office staff can assist you.

NOTE: National Cancer Institute and National Institute on Aging orders MUST be turned in on Monday by 4:30 p.m. the week before an order is needed.

All other orders MUST be turned in no later than 11:00 am each Thursday to be processed for the following week’s shipment(s).  NO EXCEPTIONS!

Please note that a 20% surcharge up to $100 will be added to cover ARP cost for procuring the animals.  Cost of freight, shipping cartons, and any miscellaneous vendor charges (for special services such as vaccinations, timed pregnant animals, etc.) are also billed to the investigators.
