Before Arrival

Residency Training Program Agreement (Contract)
Your contract with the program to which you have recently been appointed will be sent to you at the address provided on your application. Once you receive the Agreement, please review carefully and sign the document. There are two (2) pages which require signatures. Be sure to sign in both places. Return this document no later than April 24, 2009 to:

Residency Training License
You must obtain a Residency Training License prior to beginning your training program. The North Carolina Board of Medical Examiners has made the application available on-line.  Please go to:
http://www.ncmedboard.org/licensing/license_application/category/resident_training_license/.

Complete the parts on-line that you can.  Print the documents that need to be printed and mailed.  The Board has provided a section "Residency Training License Status", where you may go in after the application has been completed and submitted and check the status of your own application.   

Your on-line application will be electronically sent to the House Staff Office at this institution.  Your application will remain with the House Staff Office until all paper credentials and documents arrive from the sources in which you have requested information.  Once all required documentation is received, the House Staff Office will electronically submit the on-line application to the Board and mail the paper credentials and documents for process and review. 

If you have difficulty downloading any of the forms within the Residency Training License Application, the forms are being provided here for your convenience.

 The House Staff Office will coordinate the application process and act as liaison between you, the applicant, and the North Carolina Medical Board.  Unless otherwise indicated during the application process, all paper credentials and documents are to be sent directly to the House Staff OfficePlease do not contact the North Carolina Medical Board.  If you have any questions or concerns regarding the application process, please direct them to the House Staff Office.

 ALL Residency Training License Applications must be submitted to the House Staff Office. These applications should be complete. If your school is one that does not complete the Medical School Certification Form until after graduation, please send your application in without that form. It can be reviewed and held for issuing pending that one form. Please work to have all the other forms and documentation necessary by that time. 

Computer generated pictures submitted with the your Medical School Certification Forum  are not acceptable.  
Pictures are required to have a glossy finish and have the school seal imprinted on it.  

The license is absolutely necessary in order for you to begin training. Put this first on your list of things to do. Delays in licenses have caused many residents delays in beginning their training.
The importance of this application cannot be stressed enough. Do it timely, completely and provide all necessary documentation.

Note: If you have not yet graduated from medical school, your school must complete “Medical School Certification Form A”. If you have graduated already Form B must be completed. If you turn your form into your school and they are not going to process until after graduation, be sure you give them Form B. The Board will not accept the wrong form. Completion of the wrong form has slowed down the licensing process for many entering a training program

Identification Badges

All employees of the Medical Center must have and wear a photo identification badge. The badge acts as identification and gains you entrance into the employee parking deck. It also allows you to obtain free meals when on-call.

ID badges are to be obtained in the Medical Center ID Badge Department, Third Floor, Meads Hall prior to orientation.

Medical Center E-mail Accounts

All incoming house staff have a Medical Center e-mail account set up prior to arrival. Particular information about e-mail addresses is provided at orientation.

House Staff Mailboxes

All house staff are assigned a mailbox in the Medical Center’s Mailroom as of July 1. On or after July 1, each house officer must go by the mailroom’s service window and request his/her mailbox assignment and combination. Please do not inquire about your mailbox prior to July 1.

Exceptions: Diagnostic Radiology, Family Medicine, Anesthesiology residents receive all of their mail in departmental mailboxes.

Beepers

Beepers are assigned by the program to which you have been appointed.

Lab Coats

Lab coats are provided to incoming residents by their department.  Please check with the program in which you will be training to determine the process for obtaining lab coats.  If your program is not providing lab coats, they can be purchased in the Gift Shop, Main Floor of Ardmore Tower. 

Check with your department for details about getting lab coats cleaned.  Cleaning services are also available via the Gift Shop.

Children's Choice Learning Center (CCLC)

The Medical Center provides a child care center for the children of Medical Center employees. Children's Choice Learning Center (CCLC) accepts children between the ages of 6 weeks and 5 years.

The Center is open 6 am to 6:15 pm, Monday through Friday. The cost of placing a child in the Center is automatically payroll deducted and varies depending on the age of the child and the hours of care. To receive more information or put your child’s name on the waiting list, call 336-716-0300.

On-Call Meals and Accommodations

All house staff required to remain in the hospital overnight are provided dinner and breakfast by the hospital free of charge. Please see the On-Call Meals for Residents policy for specific information.

All house staff required to remain in the hospital overnight are provided on-call accommodations for the night. These rooms are reserved by the house officer by calling 6-0007 on the day of their call. Please see theOn-Call Rooms  On-Call Room Accommodations policy for specific information.

Pay Advance Program for Incoming Residents

The Hospital provides a Pay Advance Program for incoming residents. This program allows residents to request an advance in their contract salary up to $2,000. Biweekly checks will be adjusted accordingly. The resident must agree to repay this advance by payroll deduction beginning with their 1st paycheck in July and ending on December 31 of this calendar year. Checks will be distributed after work commences.  Residents will be notified via a page to alert you when checks are ready for distribution.  This is a one-time only opportunity to help the residents financially until the regular bi-weekly paychecks begin. 

To take advantage of the Pay Advance Program, print, complete and mail the Pay Advance Form to Diane Hartley in the House Staff Office at any time prior to orientation.

Resident Spouses Association (RSA)

A Message from the RSA President

On behalf of the Resident Spouses Association (RSA), I would like to welcome you to the Winston-Salem area and encourage you to join our organization. RSA is a wonderful way to meet other spouses who share your interests and experiences. RSA is a support group for resident families. We sponsor and organize many activities throughout the year and we try to appeal to the needs and interests of all our members. RSA organizes events geared towards both families with and without children. Some of our events include: weekly playgroups, monthly meetings, a wine and cheese party, holiday parties, game nights, and girl's night out. We also help support the Ronald McDonald house with bake sales and monthly dinners. Please email me with your name and address and I will send you a membership form and a welcome packet with information on Winston-Salem.
