The Significant Financial Interest Disclosure (SFID) process is automated and available online at the link below, via the PeopleSoft Human Resources Self-Service Menu. To login, type your MedCenter User ID and password.

Annual Disclosure
According to WFUHS policy, at least annually each faculty member and key administrator will disclose any private financial relationship with an outside entity related to their school activities that amounts to greater than $10,000 or 5% ownership interest. Related financial conflicts of interest must be managed, reduced or eliminated.
Disclosure updates and revisions
Disclosures must be kept current. When substantial changes in external activities occur, faculty and staff are required to update their disclosure in a timely manner, e.g., 14 days. Select the link above to revise your disclosure. Each disclosure submission is individually date stamped and archived.
