Registration

Before your child starts at Children’s Choice Learning Center you will be given an enrollment folder. This folder includes important information that must be completed and returned a minimum of one week prior to the start date for your child.  You will find the following in this folder:

  • Enrollment form
  • Health information and authorization form
  • Health statement
  • Acknowledgement of a special need
  • Child care tuition payment authorization form
  • Corp[orate sponsor childcare payroll deduction Authorization form
  • Corporate sponsor parental agreement
  • Weekly commitment sheet
  • Permission to release information
  • Family connections
  • Back-up feeding plan for infants
  • Positive guidance policy acknowledgement
  • NC Daycare Laws & Rules

 

When your completed folder is returned to the center a non-refundable registration fee will be paid. This file must be returned a minimum of one week prior to the date your child will enter the Center.

These forms must be in your child’s on-site file to meet certain requirements of the Division of Child Development. The information you provide is strictly confidential and will not be released without your consent. If at any time there are changes, please notify the center in writing.

It is the responsibility of the parent to provide any changes in phone numbers, pagers, or departments to the center.  It is imperative that we have the most current means of contacting the parents in case of emergency. Emergency contact numbers should also be kept up to date.
